2006 NYS CANDIDATE QUESTIONNAIRE

 The Mental Health Voter Empowerment Project serves to register, educate and encourage consumers of mental health services, and others interested in mental health issues, to participate in their democracy and vote.  As we register individuals in your community, we ask you to assist us in helping to educate them by providing them the opportunity to better understand your position on issues crucial to them.

We would appreciate it if you would take a few minutes to answer the following questions.  Please limit yourself to 100 words or less for each answer.  Any answers exceeding the 100 word limit will be ended at the 100th word, regardless of context. 

In order to guarantee the security of each survey, we have provided you with a unique user name and password, which are provided for you in your cover letter.

Answers will be provided in a Voter Education Guide that will be published on our website, made available to our affiliate network, members of the Mental Health Voter Empowerment Project, the media, and anyone who requests it.  The deadline for submission is October 20, 2006.  Any candidate who does not return the completed questionnaire by October 20th, or does not answer a particular question, will be marked “Did Not Respond” in the survey report.
