GUIDELINES FOR DEVELOPING A CMHP TEAM'S LOCAL STRATEGIC PLAN

Part II - Defining the Plan

We recommend that the entire team complete Part I prior to engaging in the second part of the team's planning process.

List the outcomes your team will accomplish during the year.

List ways in which your plan reflects multi-cultural perspectives and outreach.

Identify resources available or essential to attaining your goals.

• What resources does your team have or need to attain your goals?
• What resources does your community have to support your endeavors?
• What resources does your team need from MHANYS to support your endeavors?

List specific goals for establishing or enhancing your team's communication capabilities, including use of internet or e-mail technologies.

Identify Roles and Responsibilities for each Team member.

• Does your team need to be expanded to accomplish your goals?
• Who needs to be recruited to complete your team?

Build a Timeline for your project.

• Identify milestones to be accomplished within specific time frames.
• Identify how often your team will meet during the year to evaluate progress.
• List ways that you will gather, track and report data to reflect achievement of your goals.
